ERF description: During precommissioning efforts at Unit 2 involving hot cell a spent fuel assembly (FA) was cut instead of a dummy FA resulting in overexposure of two persons from the reactor service maintenance staff and one person from the external commissioning organization (6.32 Rem, 9.44 Rem and 13.47 Rem respectively).

BASIS FOR RATING: The event involves personnel overexposure. Criterion - on site effects. The event corresponds to level 3 since personnel exposure exceeds 50 mSv (5 rem).
