CAPSULE BREAKAGE IN CENTRAL THIMBLE

This refers to the event reported through the INES Information Service on 24 February, 1993. Investigations have been completed. Lapse/violation in following the prescriptions of the Safety Analysis report is the principal reason of failure. The event, which should be rated level 1, is placed 1 level higher because of this lapse. Necessary administrative steps have already been suggested and are being materialized to prevent repetitions. The CT has been decontaminated and cleaned and the reactor resumed unrestircted operation on 14 March. Maximum dose received by an individual has been estimated at 115.8 micro Sv and total aggregate dose (12 persons) at 740.5 micro Sv.
