From the publication:
* This chronology is based on NRC Monthly Reports, NRC Inspection Reports, Systematic Assessments of Licensee Performance (SALP), Performance Appraisal Team (PAT) Reports, Licensee Event Reports (LER), General Public Utilities press releases, and news accounts. Between October 3 and October 10, 1985 -- NRC inspectors discovered that certain company test personnel had a "relatively weak understanding" of certain aspects of test procedures. Shift briefings were not adequate to alert operators to "off-normal" conditions. Also, plant evolutions were being performed from memory instead of in consultation with procedures which "could be considered a precursor to potential problems in the future."
