EXECUTIVE SUMMARY

Based on the site visit and walkdowns, the Pantex Plutonium Environment Safety 
& Health (ES&H) Vulnerability Assessment Working Group Assessment Team 
(WGAT) considers the Site Assessment Team (SAT) report and question sets to be a 
factual assessment of the facilities, except where specifically noted in this report. 
As a result of the SAT and WGAT' s reviews, 6 ES&H vulnerabilities, (4 by the SAT 
and 2 by the WGAT) were identified for further consideration by the Department of 
Energy (DOE) Plutonium ES&H Vulnerability Working Group preparing the final 
report. All 6 vulnerabilities were discussed with the respective site team members 
and facility experts and agreement was reached. The vulnerabilities identified by 
the SAT and WGAT are described below. No ranking or priority is implied by the 
order in which they are listed. In addition the WGAT identified and included issues 
for consideration by the Pantex Plant and DOE management that are not explicit 
ES&H vulnerabilities.

Vulnerabilities identified by the WGAT and SAT are described below and are 
included as Appendix A Vulnerability Assessment Forms (VAFs). They are 
presented by review topic and facility, and do not represent a prioritization 
of the identified vulnerabilities.

General Plutonium Pit Handling and Storage

A cross-cutting review of general plutonium pit handling and storage was 
conducted. The WGAT concurs with the two vulnerabilities identified by the 
SAT on an operational accident causing pit cladding or pit tube failure and the 
need to improve expertise on pit aging and design. The WGAT has developed a 
supplemental vulnerability on pit aging concern. Further, a second vulnerability 
was developed by the WGAT with respect to pits which have been subjected to 
special testing. These vulnerabilities apply to all facilities where plutonium pits are 
handled and/or stored. These vulnerabilities are considered low probability events 
and the potential consequences are moderate with respect to the worker and 
negligible for the public. As a result of the operational accident, environmental 
consequences would be limitedto Zone 4 where localized contamination of the 
magazine and nearby soil could occur.

12-26 Bay 27

The SAT review identified potential roof collapse due to natural phenomena as 
a vulnerability in Building 12-26 Bay 27. The WGAT concurs with the SAT 
assessment. This vulnerability has a low potential for causing an ES&H event, 
with the potential for worker injury, contamination, exposure, and a localized 
release to the environment, but no impact on the public.

12-26 Vault

The WGAT concurs with the SAT assessment that there are no vulnerabilities 
specific to Building 12-26 Vault.
