The incident occurred due to grid disturbances. During first phase of disturbance generator breaker tripped. Turbine tripped on over-speed causing generator lockout. In the second phase of disturbance, Ch-B scram came which then came to alternate supply. In the third phase of disturbances, safety system Bus B lost supply deenergising safety system channel B relays. Subsequently, channel A scram appeared due to condenser low vacuum and reactor scrammed at 1623 hrs. Scram recovery actions were taken.
