On 31.7.92 at 1722 hrs unit no. 2 scrammed on PRM high flux. PSIV 2HSO/A was found in closed condition and other three PSIVs were in open condition. On investigation, PSIV 2HSO/A air tubing (copper tube 3/4 inch size) upstairs of solenoids was found snapped off at the junction of air tubing and the main air supply line (3/4 inch size). This resulted in sudden closure of the valve due to loss of air and subsequent reactor scram on PRM high flux.
