FINGERS INJURY WHILE WORKING ON PRIMARY STEAM ISOLATION VALVE

While working on Primary Steam Isolation valve 2NSO4 B a contractor's technician had the fingers of his left hand crushed due to closure of the valve. The closure took place when a close signal was given to the valve by control room engineer who was completing the process of isolation. There was a communication gap between maintenance supervisor and control room engineer regarding the requirement of keeping valve operator in open condition.
