On 5 February 1992 at 12:20 when the plant was on line over 200 days, with 100% RX power, 632 MW gen. power and 381 ppm boron, steam started to escaping on the secondary side, from the cap at the exit of high pressure turbine. Noise was heard by a technician in the turbine hall. After examination it was found, that the steam was escaping through a crack 10 cm long and up to 1 cm wide. The operators lowered the power at 4 MW per minute. The plant was off the grid at 15:26 and reactor was shut down at 15:20, and subsequently held at hot shutdown.
