ERF description: Overhaul and refuelling of unit 2. Reactor without fuel. Due to incorrect positioning of the crane in course of introducing core basket into the pressure vessel the basket knocked at thermocouples and broke 5 of them. Two fragments of connectors' covers fell down into the reactor vessel. Consequently all inside vessel equipment was removed. Fragments were found, picked up, then checked, measured and weighted. As this check was successful reactor vessel assembly could continue. Besides of some technical insufficiencies the reason was also lack of regulation to perform this work.
