STEAM GENERATOR SAFETY VALVES WERE NOT FUNCTIONING ACCORDING TO SPECIFICATIONS

A scram occurred due to a failure of a condensate pump. Analyses of the pressure versus time charts shows that the secondary pressure exceeded the expected value. During a few minutes the pressure reached almost 110%, i.e. the permitted value. It is therefore questionable whether the vales pressure release function is fulfilling specifications. The reactor is in a standby mode awaiting a throrough examination of the valves. Compressed air is used to adjust the set point of the oopening pressure of the valves. A reason for the failure is that the conversion factor from compressed air to steam was erroneously calculated, corresponding to less than 10% higher opening pressure. This is valid also for Ringhals 3 and 4.
Justification:
Defence-in-depth degradation: incident with significant failures in safety provisions.

Initiator frequency/initiator probability: high/expected (PWR, loss of feedwater flow).
