REACTOR AUTOMATIC SHUTDOWN BY SIGNAL OF NUCLEAR INSTRUMENTATION SYSTEMSYSTEM

During test operation after critical, the reactor was automatically shutdown at 1:53 a.m. on December 11. The investigation revealed that there were imperfections in a periodic test procedure of a nuclear instrumentaiton system. This resulted in the generation of a signal which led to reactor automatic shutdown. There were no releases of radioactive materials outside of the reactor facility, nor any unplanned radiation exposures received by employees.
Justification of rating:
Both criterion 1 and 2 were not relevant for the rating of the event.
Criterion 3: "expected" initator (reactor automatic shutdown) occurred, but the safety function availability was regarded as "full" during the event. The box I-A on Table II gave the basic rating of level 0. However, as a procedural inadequacy was found, final rating was uprated to level 1.
