ERF description: The reactor operator discovered that two valves in the vent pipe to the scrubber system are closed. These two valves should be open as they are in a series with a rupture disc. However, as a signal to open the valves will occur at such an accident, the deviation is regarded as small.
The reason for the wrong line-up is that after ordinary line-up the valves were tested for leaktightness. After this test the valves were not reopened.
The event was first regarded as below scale (0), but as Forsmark 2 had the same type of event on the same system 1995, the event was upgraded to INES 1.
