On 23 July 2026, a trainee assistant radiographer did not verify radiography source retraction with a survey meter while performing device setup for at least one exposure at a job site. The worker noticed that the source was unlocked after the setup was complete. The worker did not report the incident to the responsible radiographer or the radiation safety officer (RSO) and waited until the end of the monitoring period (31 July 2026) to turn in their dosimeter. Review of work documentation by the RSO determined the date of the overexposure. For the month of July, the worker's total effective dose equivalent (TEDE), as measured by the dosimeter worn on the chest, was 0.08776 Sv (8.776 rem), and the majority of the exposure was assessed to have been received on the event date. This dose exceeds the U.S. statutory annual limit for TEDE of 0.05 Sv (5 rem). EN58422
