ERF description: The coolant channel G-09 inspection was over and fuel bundles were being loaded into the channel. It was discovered that there was some obstruction in the channel which was later confirmed to be inspection head which got detached from seal plug during retrieval of inspection assembly. It happened due to worn out female thread on inspection head extension tube which cannot be inspected due to inaccessibility. Same was retrieved back using special procedure. The obstruction came to be known while loading the fuel bundles which were getting subjected to higher compressive load than Techspecs limit. Hence the event was rated at level 1 though the bundles did not have any damage as seen during subsequent inspection.
