INCORRECT SAFEGUARD REPORTING

In the middle of April 1994 it was discovered that 39 fuel rods had been incorrectly reported in the safeguard system.
The fuel rods had been disassembled from two different fuel assemblies during the outage in 1992 in order to make it possible to inspect some guide tubes for control rods.
One of the fuel assemblies was sent to the Intermediate Storage Facility for spent fuel (CLAB) at Oskarshamn before being reassembled. The other was stored in the spent fuel pool at Ringhals 2. The fuel assembly which was sent to CLAB had 11 fuel rods missing and the fuel assembly in the spent fuel pool had 28 fuel rods missing.
The mistake was discovered in April 1994 when 39 fuel rods were found in a storage cask located in the pool for spent fuel at Ringhals 2. Corrective actions have been taken in order to reassemble the fuel assembly located in Ringhals.
A Human Performance Evaluation has been performed in order to identify weaknesses in the current routines at the plant.
