IRRADIATED FUEL ROD REMAINING IN THE FLASK WITHOUT COOLING - HUMAN ERROR

CIRUS reactor was shutdown for maintenance works. On the same day the fuel rod from J position was removed from the reactor as it has
seen the required irradiaiton. Reactor was operating at 20 MWT before being shutdown. After taking the fuel rod up into the flask, while it was being moved to storage block, it was realised that both the inlet and outlet cooling water hose reels (to and from vertical flask) were not connected. Immediately these were connected and cooling water to fuel bundle was established. The rod was without cooling for 15-18 minutes.
There was no release of any radioactivity from the fuel. Calculations were made with "no water" assumption for the maximum estimated temperature of fuel and there was no likelihood of any clad failure.
Rating of the event has to be considered using clause III-3.1.3 (Event related to fuel handling). As per this clause the event could be rated at level 1.
The last sentence of clause III-3.1.3 on page 35 states that level 2 may be appropriate for events in which there is a damage to the fuel pin integrity as a result of substantial heat up of the element. The following factors were taken into consideration. The calculations showed there was no likelihood of clad failure. The fuel bundle remained in cask. There was no release of radioactivity into the area. Water sample collected from the bottom of the rod showed very little activity (110 Bq/ml). Hence the event is rated at level 1.
