ERF description: At the annual control of the lubrication grease pumps to the Emergency Core Cooling system's core spray pumps it was detected that the lubrication pumps were unable to press the grease to the bearings of the core cooling pumps.
If there was a flooding e.g. because of a pipe break, the pump bearings would have not been protected against penetrating water.
Justification of level 1: Table 1. degradation of defence in depth without an initiator. Safety function availability, adequate, initiator frequency low.
The main reason of the failure was procedural inadequacy. The procedures are corrected now stipulating more frequent tests and controls.
