FAILURE OF A CONTAINMENT ISOLATION VALVE AT A PERIODICAL TEST

This containment isolation valve is connected to the suction side of one of the core spray pumps, its normal position and primary safety function is to be open, however in certain occasions it should be closed. At this periodical test it could not close, the direct cause was not identified at once. Next day the valve stem was cleaned and after applying some lubrication oil the valve functioned satisfactorily. According to Technical Specifications (Swedish acronym STF) the reactor should have been shut down within 24 hours.
