On April 24, 2002, a hospital reported a missing brachytherapy source used in nuclear cardiology procedures containing 9.25 GBq (250 mCi) of Ir-192 seeds. The source was found on April 25, 2002, at a landfill, covered with approximately 10 feet of landfill waste. The licensee reported that the radioactive material was still in the catheter, and the catheter was unbroken. The exposure rate was reported to be 1.1 mSv/hour (110 mrem/hour) at one meter. The licensee is investigating the cause of the incident and potential radiation exposures for individuals who may have come in contact with the source and will provide a report at a later date.
