Extremity Overexposure

During an inspection conducted by the State regulator on 27 July 2010 (and reported to the NRC on 4 August 2010), it was discovered that an employee who works in the manufacturing of iodine-125 and palladium-103 brachytherapy seeds received an occupational exposure of approximately 519 mSv (51.9 rem) to an extremity (right hand) in December 2008. The individual also received approximately 167 mSv (16.7 rem) to the left hand and 6.1 mSv (605 mrem) whole body dose. The licensee did not recognize the exposure and did not report it as required to the regulator when it occurred.
