ERF description: Unit 2 was in the forced maintenance outage to replace generator exciter anchor.
Due to alignment errors at the special water treatment facility in the auxiliary building contaminated water got into the secondary circuit of Unit 2 (deaerators, etc.). Increased activity as compared to that specified in the procedure was detected only in deaerators and SG secondary side up to 3.8E-8 Ci/l (authorized value - 2E-8 Ci/l). Radiation situation in the turbine building remained within background values.
Radioactivity release to environment, contamination of the buildings, rooms and personnel overexposure did not occur.
BASIS FOR RATING: Initiator - real, possible (increased activity of the secondary water above authorized limtis).
Safety function availability - full. According to Table A2 - level 1 (A-2R). Since the potential cause of the event in safety culture deficiencies level 2 has been chosen.
